6-3400 UPP Team (Silver): Focusing on Proactive Medication Reconciliation Completion to Increase Discharges Before Noon

Improving the medication reconciliation process enabled more patients to be discharged before noon, reducing length of stay (LOS). During Q4 2017, 12 percent of patients were discharged before noon. In the first and second quarters of 2018, the team consistently discharged at least 20 percent of patients before noon. The team continues to monitor their LOS in hours to ensure there are no negative impacts.
