How to Obtain Services

To initiate palliative care services, a patient’s treating physician should request a consultation with the URMC Palliative Care team.

Patients and/or Families: Ask your healthcare provider to request a consultation.

Referring Physicians: Call (585) 273-1154 for consultation requests or more information. Referral forms can be faxed directly to (585) 275-7403.

Emergencies: For a palliative care emergency, please call the Strong Memorial Hospital Page Office at (585) 275-2222, and ask for the palliative care physician consultant on call.
