Application Process

Prerequisites for Fellowship

In order to be considered, the applicant must have:

  1. Completed a three-year ACGME accredited pediatric residency program by the beginning of the fellowship program.
  2. Three letters of reference - one from a Pediatric Cardiologist, one from a Department Chair or a Division chief and one other letter of reference.
  3. US Citizenship, Permanent Residency or a J-1 Visa. No other Visa's are accepted.

How to Apply

Applications are accepted until December 31st. Interviews are scheduled January through April. Those selected to interview will be contacted by mail or e-mail. Selections are made through the National Resident Matching Program (NRMP).

The following materials are required to complete your application. Applications will be processed only after all the materials listed below have been received. All material must be received by the December 31st deadline.

  1. Completed APPLICATION FORM. PDF

    Complete the form online, select the option to email the form directly to us or print the form and mail it with your application. If you opt to email the form, a confirmation email of receiving your application will follow shortly.

    Note: A desktop email application (e.g. Outlook) is not required, however, it is the easiest way to send the file. Please follow the directions on screen for sending via an Internet email account (e.g. Yahoo), as there are a few extra steps.

  2. Updated curriculum vitae.
  3. Recent Photograph.
  4. Foreign graduates: Include a copy of ECFMG certificate. ECFMG certification is required of all foreign medical graduates.
  5. A copy of your USMLE Step 1 & 2 Score Reports.
  6. Three letters of reference addressed to: Roger Vermilion, M.D., Program Director (mailed to Pearl Bloom).
