Thank you for choosing URMC Department of Otolaryngology (Ear, Nose & Throat). Our goal is to provide you and your family with medicine of the highest order. To make the best use of your time and our physicians we ask that you review the following information, and complete the appropriate forms listed below.
Office Hours and Availability
Our office hours are 8:00 a.m. to 5:00 p.m., Monday through Friday. We provide same-day urgent care appointments in our Clinton Woods office located at 2365 South Clinton Ave., Suite 200.
To schedule an urgent appointment, please have your primary care or referring physician call (585) 758-5700
What to Bring to Your Appointment
Please remember to bring your x-rays, CT or MRI scans, lab reports, and medical records with you if they have not already been sent to the office. If your visit requires a referral from your primary care physician, please remember to bring it along with your insurance and identification cards, and any applicable co-payment. Please arrive at the office 10 minutes prior to your appointment to complete your registration and insurance information. If you are a new patient, please bring your completed New Patient Questionnaire(s), Appointment Policy, HIPAA Acknowledgement and Involvement in Care forms with you.
Insurance or Payment Questions
We accept most major medical insurances. All co-pays will be collected on the day of service. For any services provided that are not covered by your insurer, payment is expected at the time of service. If you have questions about the health plans we accept or about our payment policies, please contact our office at (585) 758-5700.
If you would prefer to use a credit card to pay your bill, please use URMC's secure online payment option.

Allergy Testing
If you or your child's doctor is recommending that you or your child be scheduled for testing of inhalant allergies, please print and complete all three of the the following forms:
Surgery
If you or your child's doctor is recommending that you or your child be scheduled for surgery, please review the preoperative (before surgery) and post operative (after surgery) instructions before coming to your pre-op appointment. The following information is to help guide you through the surgical process.
