Your cooperation in arriving on time for your appointment helps us stay on schedule. First-time patients should arrive 15 minutes early to allow for parking and registration and to complete paperwork. Please wear clothing conducive to physical examination. Or, we will provide a gown for your appointment. We strive to provide each patient with the highest quality care. In the event that our schedule is delayed, please rest assured that you will also get personalized attention during your appointment. If you must cancel your appointment for any reason, please call at least 24 hours in advance. That will allow us to offer the appointment to a patient from our waiting list. What to Bring to Your First Appointment

Prescription RefillsBefore coming to your appointment, please check to see what prescription refills you need so that we can provide a prescription at the time of your visit. Between appointments, call (or have your pharmacist call) your doctor’s office during regular office hours for routine refills. Because controlled substance prescriptions cannot be refilled over the phone, please make sure you always have enough medication for nights, weekends, and holidays. Medical RecordsYour medical records are strictly confidential and cannot be disclosed to anyone without your written permission. The parent or guardian must sign a special written consent form on behalf of a minor patient to authorize the release of this confidential information from our practice. Outpatient Procedure SchedulingWhen you schedule an appointment for a procedure, you will receive preparation instructions as well as the location and date of the procedure. The time of the procedure will not be determined until one business day before. You will receive instructions on obtaining the time of your outpatient procedure from your doctor’s office. Note that for any outpatient procedure, you must have someone to drive you home. We ask that your driver wait on the premises during your procedure. Billing and InsuranceContact your insurance provider directly to learn the specifics of your coverage at our practice. Payment or co-payment is due at the time of your visit. We accept cash, personal checks, Visa, and MasterCard. We also accept most types of insurance, including BlueChoice, BlueShield, Preferred Care, Aetna, as well as Medicare, some commercial insurance plans, motor vehicle accident insurance and Worker’s Compensation. We will complete the necessary insurance forms and bill the insurance company for you. Please be aware that you will receive a bill for any balance or non-covered service. Insurance UpdatesPlease notify our office if you change to a different primary care or referring provider or if your insurance coverage changes.
