Appointment Scheduling and Cancellations

Patients can be self-referred. However, we see patients by appointment only. You can schedule an appointment by calling during office hours. Also, you may request an appointment by filling in our Online Request Form. We will return your call within one business day to schedule the appointment.

We will conveniently schedule your follow-up appointments while you wait.

When scheduling an appointment, please:

  • Be specific about the reason for coming in so that we may schedule adequate time for you
  • Have the name and phone number of the physician referring you
  • Have your medical insurance card available
  • Have your physician referral number available if your insurance plan requires you to have one
  • Tell us of any special needs you or the patient may have (such as an interpreter)
  • If your visit is work related, have your Worker’s Compensation carrier’s name and case number, as well as your medical insurance card when you call
  • If your visit is related to a motor vehicle accident, have your auto insurance carrier’s name and your policy number available, as well as your medical insurance card when you call

If you must cancel your appointment for any reason, please call at least 24 hours in advance. That will allow us to offer the appointment to a patient from our waiting list.

What to Bring to Your First Appointment

Rehabilitation Physician Practice

In order to treat you, we will need your medical history and information before your first appointment. Please hand-carry any recent reports such as radiological reports and test results such as nerve conduction studies, etc. pertinent to your medical condition.

For all appointments, please bring:

  • Any forms that were mailed to you, completed. See our downloadable New Patient Questionnaires.
  • A list of prior surgeries with the dates
  • A list of your current medications (with dosages), including any over-the-counter medications or supplements you have taken in the past six months. We will record this for you if you bring your medications and supplements to your appointment in their original bottles
  • Your medical and prescription drug insurance cards
  • X-rays, MRIs, myelograms, and CTs should be hand carried by you to your first appointment. If you have had these taken at University Medical Imaging, Highland Hospital, or Strong Memorial Hospital, please inform us when you make your appointment. Will obtain these for you.
  • If your visit is related to a motor vehicle accident, bring your auto insurance carrier’s name and your policy number as well as your medical insurance card.
  • If your visit is work related, please bring your Worker’s Compensation carrier’s name, case manager or adjuster’s name, phone and fax numbers, and the case number, if assigned. You will also need your medical insurance card and the name of your employer. Please be prepared to discuss how the injury occurred.

  • The Script for Physical Therapy that your referring MD has given to you.

Our Locations

Your cooperation in arriving on time for your appointment helps us stay on schedule. First-time patients should arrive 15 minutes early to allow for parking and registration and to complete paperwork. Please wear clothing conducive to physical examination. Or, we will provide a gown for your appointment. We strive to provide each patient with the highest quality care. In the event that our schedule is delayed, please rest assured that you will also get personalized attention during your appointment.

Prescription Refills from Physicians

Before coming to your appointment, please check to see what prescription refills you need so that we can provide prescription at the time of your visit. Between appointments, call (or have your pharmacist call) the office during regular office hours for routine refills. Because controlled substance prescriptions cannot be refilled over the phone, please make sure you always have enough medication for nights, weekends, and holidays.

Our staff will assist you in completing any forms sent home if you are unable. Because your comfort is important to us, our exam tables are accessible to patients in wheelchairs and those with mobility impairments. We are prepared to accommodate your needs. Medical Records

Your medical records are strictly confidential and cannot be disclosed to anyone without written permission. The parent or guardian must sign a special written consent form on behalf of a minor patient to authorize the release of this confidential information from our practice.

Billing and Insurance

Contact your insurance provider directly to learn the specifics of your coverage at our practice.
Please note that your responsibility for co-payments may change 91 days after your surgery.
Payment or co-payment is due at the time of your visit. We accept cash, personal checks, Visa, and MasterCard.

We also accept most types of insurance, including BlueChoice, BlueShield, Preferred Care, Aetna, as well as Medicare, some commercial insurance plans, motor vehicle accident insurance and Worker’s Compensation.

We will complete the necessary forms and bill the insurance company for you. Please be aware that you will receive a bill for any balance or non-covered service. Our staff will be happy to assist you by phone with any billing questions. Contact us at 275-3272.

Insurance Updates

Please notify our office if you change to a different primary care or referring provider or if your insurance coverage changes. We will need the new information printed on your insurance card.
