Preparing Your Patient for a First Visit

In order to provide the best possible treatment to your patient, we request that you provide medical information pertinent to his or her current condition in advance. Please forward to us any recent reports such as radiological or other imaging reports and test results such as nerve conduction studies, DEXA scans, etc by fax. Or, you may provide these items to your patient and instruct them to bring them to the physician's office. In addition, you may print a copy of the URMC Orthopaedics and Rehabilitation New Patient Registration form and the physician's New Patient Questionnaire for your patient. They can fill them in at their leisure and bring them along to speed the registration process.
