Refer a Patient
You may refer to a specific physician or ask our referral specialists to schedule the first available appointment for your patient. To refer your patient to our therapeutic or rehabilitative services, please print and fill in one of these forms, which are in PDF format.
Refer a Patient for Urgent Orthopaedic Care
Many of our providers can see patients on an urgent basis. In addition, dedicated urgent care is provided 8 am - 7:30 pm Monday through Friday for patients who must be seen the same day. Please call the appropriate number below so that we may prepare for your patient's arrival.
If recent imaging or other reports pertinent to your patient's condition are available, please forward them to us by fax. We can access X-rays taken within the Strong Health system from our office, or patients may hand-carry films.
