Referring Physicians
Referral Requirements for Specialty MDs
Referrals are required for all specialty MDs in the following areas:
- Cataract
- Cornea
- Glaucoma
- Low Vision
- Ocular Plastics (coming soon)
- Pediatric Ophthalmology
- Uveitis
Additionally, the following departments also require provider review after receiving clinical documents:
- Retina
- Neuro-Ophthalmology
- Genetics
- Ocular Oncology

Referrals to Neuro-Ophthalmology
Referral Review Process
All internal and external referrals to Neuro-Ophthalmology must include the Neuro-Ophthalmology Questionnaire Referral Form.
The form must be completed by the referring provider/office and submitted with the referral before it can be reviewed by the Neuro-Ophthalmologists at FEI.
Required Documentation for Neuro-Ophthalmology Referrals:
- Completed Neuro-Ophthalmology Questionnaire Referral Form
- Relevant clinical documentation and office notes
- Supporting diagnostic testing and records, if applicable
Note: If a referral is received without the completed questionnaire, it will be documented and closed until the required form and supporting documentation are submitted.

Referrals to Other Departments
For referrals to departments other than Neuro-Ophthalmology, please include the following:
- Patient Information:
- Full Name (include parent/guardian name if the patient is a minor)
- Date of Birth
- Best phone number to reach the patient, parent, or guardian
- Confirmed current address
- Insurance carrier and subscriber ID
- Clinical Documentation:
- Recent eye notes (preferably <1 year old) reflecting the diagnosis for which the referral is being made
- Any Visual Fields/OCT Testing
- Color images must be mailed to 175 Corporate Woods, Suite 230, Rochester, NY 14623
- Urgency:
- Clearly mark the referral as URGENT if applicable

Diagnoses That Do Not Require a Formal Referral
For the following diagnoses, patients can schedule directly by calling (585) 273-3937:
- Routine Eye Exams
- Diabetic Eye Exams
- Longstanding Blurry Vision
