Contact Lens Order Form

We offer a convenient way to order contacts. Use the form below and we will provide you with a quote for our lowest price on your lenses within two business days. We'll also check your insurance and look for special rebates to save you even more. Please be aware that this form is not for re-ordering specialized medical contact lenses such as scleral lenses or PROSE.

One-year supply orders qualify for FREE shipping. Shorter-term orders may be subject to shipping and handling charges.

New Payment Policy

The University of Rochester will no longer collect personal credit card information over the phone. 

To protect your privacy, payments can now be made in one of the following ways: 

• Card on File – We can process your payment using the card saved in your MyChart account.
• Secure Payment Link – We can text you a secure link to enter your payment information directly in Epic. To receive this link, your personal phone must accept text messages from the University. Without this permission, we cannot process your payment over the phone. 
• In Person – You may pay directly at any Flaum Eye Institute Optical Shop location.
