Our Services

At Partner Residential Facilities

We make routine visits to our patients at partner nursing homes and senior living communities, and we coordinate their medical care to ensure that we have complete information. We collaborate with social workers, physical and occupational therapists, the staff of senior living facilities, and other health care providers to offer efficient and comprehensive services to patients right where they live.

  • Blood draws to complete needed lab work
  • Mobile X-rays
  • Physical therapy evaluation and services
  • Occupational therapy
  • Wound assessment and treatment
  • Risk assessment for falls
  • Evaluations for dementia, Alzheimer’s and depression
  • Dietary and nutritional assessment
  • Medication review and reconciliation
  • Referrals to specialists for new or pre-existing conditions
  • On-call service for unexpected medical needs

Hospital Care

When hospital treatment is needed, our physicians can admit to both Highland Hospital and Strong Memorial Hospital. We admit most of our patients to Highland because it offers the most comprehensive and innovative Geriatric Services in the region, and it is renowned for compassionate care in a comfortable community hospital environment.

  • Highland has more board-certified geriatricians on staff than all other area hospitals combined.
  • The staff in the Highland Emergency Department is trained to be aware of the special needs of older patients, and the department offers streamlined services and elder-friendly features.
  • Highland's direct admission process simplifies admission for patients arriving from nursing homes or senior living communities. In many cases, the patient is able to skip the Emergency Department and be directly admitted to the Acute Care for Elders unit.
  • The Acute Care for Elders unit is collaborative health care environment where geriatricians, nurses, physical therapists, occupational therapists, social workers, nutritionists and pharmacists all work together to care for patients.
  • Highland is a New York State designated Stroke Center. A response team, headed by a board-certified neurologist is available 24 hours a day, 7 days a week, and sophisticated brain imaging and monitoring equipment is on-site.
  • Geriatric Fracture Center is dedicated exclusively to the care of elderly patients who suffer fractures. The Center takes an advanced approach to the treatment of fractures in older adults, combining the care of an orthopaedic surgeon with that of a geriatrician. This collaboration ensures better outcomes.
