Health Home Fact Sheet for Children and Families
The Health Home Program
In New York State, many people get their health benefits through the Medicaid Program. Some children have chronic health problems, developmental delays or mental health needs and can’t always manage all of their medical and day to day needs. New York State’s Health Home program was created with these people in mind. The goal of the Health Home program is to make sure its members get the care and services needed.
After Enrollment
Once you are enrolled in the Health Home program you will have your own care manager. The care manager will work with you to set up a care plan, schedule appointments if you need and to help you and your family obtain the services necessary to better meet your child’s needs. Some of the services care managers provide are:
- Linking to community programs
- Scheduling appointments
- Obtaining transportation to medical and non-medical appointments
- Assisting with housing needs
- Attending medical appointments for your child
- Communicating with all providers involved with the child’s care
- Locating resources for food, clothing, social services, financial needs
Cost to Enroll
The Health Home Program is provided free of charge to you if you are on Medicaid and qualify for the program based upon the criteria, which can be determined by the social worker or someone else in the doctor’s office.
Enrollment Timeframe
You can enroll in the program at any time. You can also decide to withdraw from the program at any time as well.
Requirements
The Health Home program asks that you remain in contact with your care manager as needed. Your care manager will make phone contact and meet with you in person as needed. The care manager is there to provide you and your child with the resources that are needed to be healthy. The goal is to work together as a team with the child, their family, all of the child’s providers and the care manager.
