If your child is enrolled in the Children's Health Home Program you will have your own care manager. The care manager will work with you to set up a care plan, schedule appointments if you need and to help you and your family obtain the services necessary to better meet your child's needs.

• Scheduling appointments
• Obtaining transportation to medical and non-medical appointments
• Assisting with housing needs
• Attending medical appointments for your child
• Communicating with all providers involved with the child's care
• Locating resources for food, clothing, social services, financial needs
