Overview

Explore the social determinants of health for children and families in the City of Rochester and Monroe County, and the resources that address them.

Learning Objectives

  • Describe and give specific examples of how education, poverty, cultural background, race, and other social determinants affect individual and community health status.
  • Demonstrate a working knowledge of community resources and develop competence in connecting patients and families to these resources.
  • Develop proficiency in communication skills to better facilitate the learning of patients, families, and community members.
  • Apply practical advocacy skills.

  • If you want to visit additional community-based organizations not on your schedule, PLC staff can help with providing contact information

Required Conferences

  • First Wednesday of rotation: Orientation
  • First Tuesday of rotation: Guided Discussion
  • Last Tuesday of rotation (unless you are an R2, then date/time TBD): Closing

Rotation Expectations

  • Attend all scheduled site visits at scheduled times.
  • Review the organization’s website/PLC Pedia listing prior to visiting for background info.
  • Be prepared to ask questions about the organizations/programs you are visiting.

Required Assignments

Complete and email the following to the Faculty Leader and Rosa one day prior to the PLC
closing:

  • Reflection piece (paragraph, poem, collage, photo, art piece, playlist, etc.)
  • Exploratory Learning activity and write-up
  • Bus transportation and grocery store exercise and write-up
  • PLC Site Evaluation form
