Program Goals

  • Connect, complement, and extend a single system of care for Rochester youth
  • Build trust and engagement to strengthen relationships with parents, children, medical providers/practices, schools, and school nurses
  • Involve school nurses in/across medical care systems/settings to benefit RCSD students' education and well-being outcomes
  • Enable school nurses to work to their full ability, role, and skill set on behalf of the children they care for
  • Make it easier for school nurses to help parents care for their children

Who is This For?

  • RCSD students (under 18 years) 
    • Who attend ANY Rochester City School District (RCSD) school
    • Followed by ANY Finger Lakes Region medical practice, specialty, or healthcare system
    •  With ANY medical condition or health need, or none at all
  • Smarter Student Health is currently not available for students outside RCSD, or who attend charter or private schools

How the Program Works

With parent permission, school nurses can:

  • View a student's electronic health record within the Rochester RHIO (Health Information Exchange) and connected with any Finger Lakes Region health system, medical home and/or specialty practice
  • Access care plans and details about healthcare visits—this includes care plans for asthma, allergies, ADHD, diabetes, concussions, and seizures; prescription medicines; and lab results
  • Securely send and receive messages directly to/from a child's healthcare team (in development)
  • Note when a healthcare provider documents that a child may miss school or PE/gym

The more school nurses know, the more they can do to help parents and their children. A parent's role is very important, and we encourage parents to communicate regularly with their child's school nurse. Tell them right away when your child's health, care plan or medicines change, or if there is anything else they should know.

You may also ask your RCSD school nurse or your healthcare  provider for the two required enrollment forms.

The two enrollment forms are available in additional languages. Please inquire about this. 

Your child's information within the Rochester RHIO is secure. Only approved and licensed RCSD school nurses can access your child's health information.

Smarter Student Health enrollment will remain in place until your child no longer attends RCSD (or until age 18), unless you withdraw from the program in writing.
