Call Transfer Center at 1-800-499-9298
- Conference Call with transport team and attending/fellow
- Information needed for conference call
- Patient name, DOB, Age, Weight, Allergies
- Brief description of present illness and presentation
- Full set of vital signs
- Any laboratory and imaging studies done
- Medications given
- Brief assessment
- Transport decision made including mode of transport (Ground, Helicopter, Fixed Wing)
- Have summary of all notes, labs, and vital signs printed and ready for team when they arrive
