  • Reach Out and Read
    Reach Out and Read is a nationally supported program that provides a book at each well visit for children ages 6 months to 5 years old.
  • Dolly Parton’s Imagination Library
    Dolly Parton’s Imagination Library – A literacy program available to patients residing in the 14605 zip code. Once families are enrolled in the program, the child will receive a developmentally appropriate book mailed to their home each month until they are 5 years old.
  • Healthy Families Monroe
    Healthy Families Monroe is a home visiting program for pregnant women or moms who have an infant under the age of 3 months. Highly trained Family Support Specialists provide home visitation, developmental assessments, evidence-based parenting, family goal setting and connections to support services.
  • Teenage Parenting Support Services (TAPSS)
    TAPSS provides home-based, comprehensive and intensive supportive counseling and case management for any young parent (male or female, individually and as couples) who resides in Monroe County, is pregnant or parenting, and is under 22 years old. Referrals often center on basic needs, child care, domestic violence, mental health counseling, early intervention, education and employment, housing and medical services.
  • Healthy Moms Program
    The Healthy Moms Program is focused on creating a nurturing and supportive environment that is life-changing and empowers women to reach their highest potential. The program provides moms with the support they need to be physically healthy, emotionally healthy, and self-sufficient. Services provided include behavioral health support, health home care management, support with education and career objectives, smoking cessation program, and family connects program.
  • The Early Intervention Program (EI)
    The Early Intervention Program is a voluntary developmental evaluation and services program offered by New York State and administered locally by the Monroe County Department of Public Health. EI serves children from birth through 2 years of age and their families where there is a high risk of delay, a suspected delay in development, or a confirmed diagnosis of developmental disability. Evaluations are conducted to determine eligibility. A variety of therapeutic and support services are offered to eligible infants and toddlers with disabilities and their families.
    If a child is found not eligible, parents can request developmental monitoring.
  • SPCC and Mt. Hope Family Center
    Families who have active CPS involvement may have access to additional community programs through SPCC and Mt. Hope Family Center.
  • Car Seat Education - healthychildren.org

    This federal program offers information on how to receive assistance. The goal of this federal program is to safeguard the health of low-income women, infants, and children up to age 5, who are at nutrition risk by providing nutritious foods to supplement diets, information on healthy eating, and referrals to healthcare.
