ACGME Accredited Training Verification Requests
Training Verifications
The Graduate Medical Education (GME) Office completes the following training verifications for all residents and fellows who have participated in an ACGME-accredited residency or fellowship program:
- Training dates verifications
- Detailed training verifications for one or more programs
- State licensure verifications
Processing time: Requests are generally completed within 10 business days. During the busy season (May–September), please allow up to 30 days for verifications.
Verification Fees
When is there a fee? A fee applies when the physician’s training was completed more than 12 months ago. The GME Office does not process requests until the fee has been collected. Once the request form is complete, you will be prompted to pay the fee via credit card.

Required Authorization and Release
We require a signed Authorization and Release form for all verification requests. You must upload this form during the request process or send it via email after payment has been made.
If a signed Authorization and Release form is not provided, we will not be able to complete your verification.
