Health Home

Health home is not a building; it is a new care management model designed
by Medicaid to help high risk patients.

A health home consists of providers and community agencies that collaborate to provide services to people with Medicaid coverage who have a serious mental illness or two chronic conditions. It enables patients to be healthier and safer in the community.

Nurse-Family Partnership

The Nurse-Family Partnership is an evidence-based home visitation program that improves the health, well-being, and self-sufficiency of low-income, first-time parents and their children.
