Eligibility

Your providers will assess if you are a good candidate for Hospital at Home. There are several factors involved in the assessment related to your condition and your living situation. You must be 19 years old or older, have a home that meets the criteria for a safe environment in which to recover and receive treatment, and live in Monroe County within 30 minutes of the hospital campus. Other factors in the assessment include insurance eligibility, active and supportive care givers to assist you, and meeting the criteria for hospital medication management.

Hospital at Home patients receive hospital-level care in the comfort of their own home. They enjoy greater flexibility in visitation and routine with the same assurance of safety and quality of care as they would receive while in the hospital.

Patients must be seen in the Strong hospital emergency department or as an inpatient. If you and your provider both decide Hospital at Home is right for you, a care plan will be developed for your treatment at home. Only patients who meet the criteria and are clinically appropriate will be recommended.

Typically, you will have two visits daily from nurses— one in the morning and one in the afternoon— and a telehealth visit from your physician once a day with the nurse present. You will have an overnight care team assigned to your care.  Depending on your condition, you may also have visits from specialists such as physical, occupational, and other therapists. 

When you first arrive, a nurse will set you up with a WiFi hot spot to ensure reliable communication and a tablet with a call button with which you can reach your care team 24/7.

If you desire meal service, your nurse will arrange for Meals on Wheels to deliver to your home and will make sure you have enough food to last until the first delivery.

If you qualify for UR Medicine Hospital at Home, there is no additional out-of-pocket cost compared with what you would pay in the hospital. The cost to you is determined by the services provided to you and the coverage of your insurance plan.

The level of activity you are allowed depends on your individual condition and health status. Just as if you are admitted in the hospital, you are expected to remain at home until your physician or care team determines you can resume normal activities so you can receive treatments and be in close communications with your care team.

We understand your pets are a positive source of comfort and companionship. All we ask is that you secure your pet in another room during visits from your care team members. This allows them to provide you with their undivided attention during consultations.
