Finger Lakes Health's Seneca Family Health Center is located at 367A East Main Street, Waterloo, New York 13165. Dr. Luke Chen, Dr. Sixto Medina, Dr. Eric Shives, Christina Gray, MSN, ANP, CNS, Stephanie Humbert, MSN, FNP-BC, Susan Randall-Mantella, MSN, FNP-C, Melissa Sherman, MSN, FNP-C, Elizabeth Smith, MSN, FNP-C and Jennifer Whitmore, MSN, AGPCNP-BC practice family medicine at this site, providing both adult and pediatric care.
Seneca Family Health Center is currently accepting new patients. Finger Lakes Health's Family Health Centers do not require a referral. Patients normally enroll in the practice, which is often required by insurance carriers. The practice can assist you with this process. Appointments are preferred and most health insurance plans are welcome. Cash, checks and major credit cards are also accepted.
We offer Telemedicine as an alternative to coming into the office. Telemedicine is appropriate for many conditions but not all.
