Discharge Planning & Case Management
St. James Hospital’s Case Management department offers free discharge planning and social work services to patients and their families.
Our Case Managers help individuals with many different social, psychological or environmental needs that arise due to a patient’s illness, diagnosis or treatment.
As licensed registered nurses, our Case Managers understand a patient’s many needs, and act as a liaison between the hospital, patient, family, and community agencies. They also serve as a resource to other hospital departments and physicians to assist with long-term care placement, home care, insurance, medical equipment, rehab services, hospice, financial and protective issues, and more.
If you need assistance, contact our Case Management department at (607) 247-2363, Monday through Friday, 6:30 a.m. to 4:00 p.m. Additional information on local resources is available in our Discharge Planning booklet. Let us know how we can help you and your family.
