Today, more than 90% of children with special health care needs survive into adulthood due to advances in medical care and the early intervention strategies provided by pediatric providers. In fact, adult patients with chronic conditions of childhood origin are now outnumbering pediatric patients among some subgroups. Thus, there is an urgent need to provide the same high quality care for this target population in the adult health care system.
