  • Ayat Alamiri, DA and Steinbacher, Maureen, CDA, are dental assistants providing assistance to our dentists and dental residents.
  • Heather Busick, MD provides primary care, physicals, medication refills, and referrals for our patients.
  • Leanne Cassevoy as the Dental Treatment Coordinator manages the billing, registration operations, case management, dental data practice reporting and provides administrative assistance at the CCC.
  • Kristie Cowens serves as the Prior Authorization Specialist (PAS) who obtains prior authorization for enteral products, medications, procedures, hospitalizations, and durable medical equipment (DME).
  • Francis Coyne, MD provides primary care, physicals, medication refills, and referrals for our patients.
  • Sarah Creek, RN, Andrea Barry, RN, Emily Stender, RN prepare patients for their visits, triage phone calls, and provide condition-specific patient education.
  • Kristen Davidson, PhD, manager of the Behavioral Health team at the CCC is a clinical psychologist who provides psychological testing and/or counseling to our patients.
  • Regina Dennis is the Manager of Community Care Management.
  • Amanda Devine, MS, RN, FNP-BC is the inpatient nurse practitioner providing care for Complex Care Center patients who require hospitalization.  In addition to acute inpatient care, she allows for continuity of care for patients bridging the gap between outpatient and inpatient care along with discharge planning and coordination with outpatient services and office providers and staff. 
  • Alison Diehl, MS is the Research and Grants Administrator for the CCC.
  • Donna Germuga, RRT is the respiratory therapist, providing lung function testing and helping our adults learn more about airway clearance methods, nebulized therapies, and respiratory equipment issues.
  • Crystal Hounshell, Brittany Muoio, & Stacey Smith are integrated care coordinators who work in partnership with individuals with intellectual and developmental disabilities(I/DD) and their circle of supports to coordinate care and services needed to assist individuals achieve optimal health, wellness, and life goals. The care coordinators are responsible for providing services which include comprehensive care management, care coordination, and health promotion, comprehensive transitional care, individual and family support, referral to community and social support services, and use of Health Information Technology to link services. Care coordinators provide all services with a person-centered approach that align with NYS Office of People With Developmental Disabilities (OPWDD) Valued Outcomes.
  • Jeffrey Iler, MD, is a psychiatrist who provides behavioral health services to our patients.
  • Amy Jerum, DNP, CPNP-PC, PMHS, Director of Workforce Development, is responsible for developing a workforce climate that drives the attainment of current and future goals for both providers and those affected by complex pediatric onset-conditions.
  • Rezarta Lako, PharmD provides personal pharmacy care to identify, prevent and manage side effects of patients' medication, coordinates care with physicians, and monitors response to medication.
  • Amanda Laprime, PhD, BCBA-D is the Applied Behavior Analyst (ABA) Integrated Care Team (AICT) Director.  She offers direct ABA services to patients with autism spectrum and related disorders (ASD).  Dr. Laprime provides consultation at Golisano Children's and Strong Memorial Hospitals for in-patients experiencing behavioral challenges and education and training for medical and community providers.
  • Hannah Lease, Jamie Mussaw, & Lauren Snyder are the outpatient access specialists (OAS).  They greet patients, schedule appointments, and provide appropriate connections to a member of the team.
  • Ariadne Lie, MD, the Medical Director, provides primary care, physicals, medication refills, and referrals for our patients.
  • Alisa Malave serves as the highly functional and independent program resource for general administrative support as needed to Dr. Tiffany Pulcino.
  • Yvette Myhand, the Health Home Care Manager provides comprehensive care management services to adults over 19 years of age assigned to the Complex Care Center (CCC) by the Greater Rochester Health Home, the Health Home of Upstate NY, or providers within the center. Consistent with Medical Home and NYS regulations and policies for the provision of health home services, the Care Manager communicates with patients, physicians, and community agencies regarding patient compliance, service and care plans and provides assistance where needed.
  • Britiney Neikam, RD assists patients with creating a personalized nutrition plan to support a healthy lifestyle.

  • Leona Oakes, PhD is a clinical psychologist who provides psychological testing, counseling, and behavioral interventions to our patients.
  • Adela Planerova, DDS, is the Director of Dental Care at the CCC and along with Szilvia Arany, DMD, PhD, Leslie Schiff, DDS, Tejal Shastri, DDS provides primary dental care for patients and oversight of hygienists and residents.
  • Melanie Porcelli, RDH is a dental hygienist who provides dental hygiene care for our patients.
  • Tiffany Pulcino, MD, MPH, FACP, the Chief of the Division of Transitional Care Medicine, provides primary care, physicals, medication refills, and referrals for our patients.
  • Julia Rawlins, MSSW, is the Program Manager of Community Outreach and Care Management.  Julia leads the care management team, provides direct social work support for patients of the CCC, and interfaces with Community Based Organizations.
  • Kristine Reinhardt, MS, FNP, is available to all CCC patients on an as needed basis for illness or follow up in the office. Kristine also rounds on CCC patients in the hospital as an advocate and liaison between inpatient and outpatient teams to help coordinate care and assist with discharge planning.
  • Chadwick Roy, MBA, is the Director of Finance and Administration for the CCC and provides advice on administrative and fiscal activities especially in terms of strategic planning and problem resolution.
  • Thomas Santillo, MPH, serves as the lead administrator for the CCC which includes providing supervision, evaluation and assignment, management of all clinical functions, including all regulatory policies.
  • Steven Scofield, MD, FAAP, specializes in the care of individuals diagnosed with Cystic Fibrosis and provides primary care, physicals, medication refills, and referrals for our patients.
  • Angela Swagler, Ambulatory Technologist, rooms patients, obtains vitals, performs blood draws, assists with patient procedures, and follows up with specialty pharmacies for durable medical equipment.
