If your loved one is referred to a skilled nursing facility upon discharge from the hospital, you will choose at least 10 preferred facilities from a list of Skilled Nursing Facilities in Monroe and surrounding counties. All referrals are sent simultaneously.
Costs will vary based on insurance coverage and facility. You will work closely with a Social Worker to confirm financial information and insurance benefits.
Once a facility offers admission, we will assist you with coordinating all aspects of your discharge, including arranging transportation. Please be prepared to proceed with discharge the same day of the facility offer or the next day.
