The Transfer Center Nurse Coordinator will connect referring providers with attending physicians from the University of Rochester Medical Center and/or Highland Hospital.

Other Services
In addition to expediting patient transfers, the Strong Consult and Transfer Center will assist physicians after hours with:
- Scheduling a physician-to-physician consultation
- Information about University of Rochester/Strong Health clinical services
- Information about patients referred to Highland or Strong Memorial Hospital
Nurse coordinators will:
- Help identify which service should be contacted to assist referring providers with medical consults or potential transfers
- Request the patient's demographic information be faxed to the Transfer Center (585) 256-3156
- Arrange a conference call (call may be recorded) with an attending physician to discuss possible consult to transfer
- Secure an appropriate inpatient bed at either Strong Memorial or Highland Hospital
- Facilitate nurse-to-nurse report prior to transfer
- Coordinate critical care transport as needed for burn, cardiac and trauma patients
