Provide comprehensive medical care to the vascular surgery patients admitted to the hospital following a multitude of procedures including bypass surgery, endovascular aortic aneurysm repair, lower extremity amputations, and lysis therapy. Work with a multidisciplinary team to aide in coordinating discharge planning. Work alongside consultation services including endocrinology, infectious disease, nephrology, and physical medicine and rehabilitation to optimize patient care.

NURSE PRACTITIONER, HOSPITAL MEDICINE DIVISION, URMC

Provide comprehensive medical care to inpatients with complexity of medical diagnoses and comorbidities. Responsibilities include daily rounding, medication reconciliation and adjustment, specialty consults, admission, discharge, and cross coverage throughout day and off shifts. Work closely with attending providers, nurses, care coordination, and social work to acute medical issues alongside discharge planning.
