Providing 24-Hour Hospital-to-Hospital Consults and Transfer Services for Strong Memorial Hospital and Highland Hospital

To arrange for either a consult or a transfer of adult medical/surgical patients or pediatric (no newborns) patients, call the Consult and Transfer Center at:

The Transfer Center Nurse Coordinator will connect referring providers with attending physicians from the University of Rochester Medical Center and/or Highland Hospital.

Nurse Coordinators Will:

  • Help identify which service should be contacted to assist referring providers with medical consults or potential transfers
  • Request the patient's demographic information be faxed to the Transfer Center (585) 256-3156
  • Arrange a conference call (call may be recorded) with an attending physician to discuss possible consult to transfer
  • Secure an appropriate inpatient bed at either Strong Memorial or Highland Hospital
  • Facilitate nurse-to-nurse report prior to transfer
  • Coordinate critical care transport as needed for burn, cardiac and trauma patients

  • Information about University of Rochester / Strong Health clinical services
  • Information about patients referred to Highland or Strong Memorial Hospitals
