What We Offer
We know that survivorship can bring many changes and questions and that’s why the Judy DiMarzo Cancer Survivorship Program is here. The program provides dedicated visits, support services, and education that complements your ongoing care with your oncologist.
What to Expect from Your Survivorship Visit
A survivorship visit focuses on keeping you healthy by addressing many physical, emotional, and social complications that can come during and after cancer and its treatments. Through education and support, you’ll be empowered to take control of your health and prevent or manage potential issues.
Regardless of whether you were treated for cancer at the Wilmot Cancer Institute or at another medical facility, you can meet with our providers for a comprehensive survivorship visit.
Survivorship Care Focuses On:
- Understanding Your Cancer Journey
We’ll help you make sense of your diagnosis and the treatment you received. You’ll receive a personalized Survivorship Care Plan that outlines your care history and next steps. - Planning for What’s Next
As you move forward, we’ll guide you to helpful programs, services, and resources. A needs-based assessment helps us tailor support to what matters most to you. - Managing Long-Term and Late Effects
Cancer treatment can have lasting impacts. We’ll talk through potential long-term and late effects and offer strategies for managing them. - Recognizing Signs and Symptoms
You'll learn what symptoms to watch for and when to reach out, so you can feel confident in managing your health. - Focusing on Health and Wellness
We support healthy lifestyle habits—like nutrition, physical activity, and smoking cessation—to help you feel your best and reduce your risk of recurrence or other illnesses. - Supporting Emotional and Social Well-Being
We offer counseling, connect you with support groups, and provide resources to help you cope and thrive. - Coordinated, Ongoing Care
We work closely with your primary care provider and other specialists to make sure your care is well-coordinated. We also encourage you to be an active partner in your health journey.
Based on your preferences and personal needs, you may continue follow-up care with your primary oncology team, have annual visits in the Survivorship Clinic, or return to your primary care provider.
What is a Survivorship Care Plan?
Your Survivorship Care Plan is a personalized document that serves as a roadmap for life after cancer treatment. It provides a foundation for ongoing conversations about your long-term health and wellness. It includes:
- A summary of your treatment, including your care team, diagnosis, and types of therapy received
- Information on side effects that you may be experiencing now or could develop in the future
- Recommendations for follow-up care and future screenings
- Support resources for emotional and mental well-being
- A list of helpful local, regional, and national support services
- Programs to support healthy living—like nutrition, exercise, mindfulness, smoking cessation, and support groups
It is important to see your primary care physician (PCP) for regular visits. Find PCPs currently accepting new patients.

If you don't see a clinic for your specific type of cancer listed here, don't worry - your primary oncology team will continue to provide you with the necessary survivorship care. We are continuing to expand our clinics to support individuals with a broader range of cancer types.
