Secure Document Upload

University of Rochester School of Medicine and Dentistry Financial Aid Office provides a secure file upload system to submit your information and safeguard your personal data. If you would like to upload information to our office securely, please use the secure document upload form below. The more student specific information you can provide us (i.e., student name, URID/AMCAS number/last 4 digits of your Social Security number, etc.) the quicker we can add the information to your file.

When submitting documents, please select the document type from the dropdown list, and then attach your document. If the document you are submitting does not appear in the dropdown list, simply select ‘other’, and describe the document in the comment box. You can submit up to 5 documents at a time. If you need to submit additional documents, simply reset the form and submit your additional documents.

Only files with the following extensions (".pdf", ".png", ".jpg", ".jpeg") can be uploaded.

Please do not email sensitive information such as tax returns listing social security numbers or other personally identifiable information.


Note: Files can only have one of the following extensions (".pdf", ".png", ".jpg", ".jpeg") and can be no more than 2 MB in size.
