Residents have their continuity practice for all three years at either Highland Family Medicine or Brown Square Community Health Center. Each site provides residents with a team-based group practice experience. The hospital settings are a mix of university-based and community-based models.
Tri-County Family Medicine – Dansville, NY

Tri-County Family Medicine (TCFM) is an FQHC-Look-Alike (FQHC-LAL) organization with 6 health centers distributed throughout Allegany, Livingston, and Steuben counties in the Finger Lakes area of Western New York. TCFM has provided comprehensive care for these communities for over 50 years, providing high quality primary care that includes pediatrics, women's health and obstetrics, and office procedures. TCFM also offers integrative pharmaceutical care with a dedicated pharmacy within one of its sites. Almost 20,000 patients receive their care/visits occur through the system with over 7000 per year at the main residency site.
Noyes Hospital (NNH)

Noyes Memorial Hospital is a 67-bed community hospital in Dansville, NY, affiliated with the University of Rochester Medical Center (URMC) and serving Livingston, Allegany, Steuben, and surrounding counties. It is one of the key home hospitals for the Rural Family Medicine Residency Program. Residents rotating through Noyes will gain hands-on experience in hospital medicine, ICU care, obstetrics, and outpatient specialties, benefiting from the hospital’s blend of community-focused practice and academic affiliation.
Noyes traces its origins back over a century, beginning as Dansville Medical and Surgical Hospital in the 1860’s. After evolving through several incarnations and community efforts, it became known as Nicholas H. Noyes Memorial Hospital in 1972 in recognition of significant philanthropic support from Nicholas H. Noyes, a local native whose foundation has supported the hospital’s growth and modernization.
Today, Noyes is a part of a comprehensive healthcare system providing not only acute care but also an intensive care unit, advanced interventional radiology and imaging, a 24-hour emergency department, surgical services, orthopedic care, mental health services, specialized diabetes and stroke programs, and a cancer center in partnership with UR Medicine’s Wilmot Cancer Institute.
The hospital also supports a broad range of outpatient clinics in conjunction with Tri- County Family Medicine and leads community health initiatives, helping ensure patients receive high-quality care close to home rather than needing to travel to larger regional centers.
St. James Hospital (SJH)

St. James Hospital is a 20-bed community hospital located in Hornell, NY, and affiliated with the University of Rochester Medical Center (URMC). It serves rural communities in Steuben County and surrounding areas and will be another home hospital for the Rural Family Medicine Residency Program. Residents at St. James rotate through hospital medicine, emergency medicine, general surgery, orthopedics, and other sub-specialties.
Founded in 1890, St. James began as a small rural healthcare facility and has steadily evolved to meet the growing needs of its community. Over its long history, it has expanded services and modernized its facilities to offer comprehensive inpatient and outpatient care, including a 24-hour emergency department, general surgery, orthopedic and rehabilitation services, imaging and laboratory services, urgent care, and various outpatient specialty clinics.
In recent years, St. James has invested in campus renovations and new construction, including an updated medical office building and state-of-the-art care spaces designed to provide high-quality health services locally rather than requiring travel to distant academic centers.
The hospital plays an essential community role by providing accessible care, supporting chronic disease management and preventive health initiatives, and partnering with URMC to enhance specialty services in a rural setting.
Highland Hospital (HH)
Highland Hospital is a 319-bed community hospital affiliated with the University of Rochester. It is the urban Family Medicine program's home hospital. Rural residents rotate on the hospital medicine and OB services.
Strong Memorial Hospital (SMH)
Strong Memorial Hospital is an 886-bed tertiary care center that is the main teaching hospital for the University of Rochester School of Medicine and Dentistry. It is the primary site for residency programs in most fields. The medical school library is located in the hospital. Training at Strong includes 4 weeks of inpatient pediatrics and 2 weeks of NICU.
Rochester General Hospital (RGH)
Rochester General Hospital is a 528-bed private teaching hospital with an independent residency program in OB/GYN and a university program in pediatrics. It has a busy emergency department and a Level II nursery. Residents do 4 weeks of rotation in the pediatric emergency department and 2 weeks of inpatient pediatrics.
