Mayewski Professor of Medicine at URMC. He is also the Medical Director of the faculty-resident internal medicine practice at Strong Memorial Hospital. He is the Co-Chair of the Clinical Quality Committee of the Finger Lakes Preferred Provider System, a thirteen county organization which is part of the New York State initiative to improve the quality of care for Medicaid patients. Dr. Berliant attended medical school at the University of Illinois School of Medicine and completed his internal medicine residency and chief residency at URMC. He has led several hospital and community initiatives to enhance the safety and quality of care for patients in the Rochester area. Dr. Berliant’s professional interests include new models of health care delivery and improving equity in health care.Professional BackgroundDr. Berliant pioneered the Virtual Care Center model of care for the University of Rochester Medicine Service. This care model provided intensive virtual follow up to high-risk patients following hospital admission and discharge. The program, which was funded by the Greater Rochester Health Foundation from 2013-2016, emphasized better post-discharge care coordination and multi-disciplinary patient management in the transition from hospital to home, which are significant contributors both to poor health outcomes and costly (and avoidable) hospital readmissions. Dr. Berliant was also the Director of Safe Transitions, a Multi-disciplinary Project to Reduce Hospital Admissions at the University of Rochester, named by UHC/AAMC as top performer in “Best Practices For Better Care,” with reductions in 7 day hospital readmissions of 18% over 15 months. Most recently, he has been collaborating with Drs. Mammen, Arcoleo, and Halterman in pioneering new approaches to primary care asthma management through the TEAMS asthma telemedicine program. Dr. Berliant was co-PI for a regional New York State Hospital-Medical Home Demonstration grant that promoted improved care coordination, patient continuity and quality of care for Medicaid beneficiaries. Outpatient primary care training sites were transformed into high functioning Patient Centered Medical Homes as recognized by the National Committee on Quality Assurance. Dr. Berliant is active in the development of the ambulatory educational curriculum for the Internal Medicine Residency Program at Strong Memorial Hospital. He teaches primary care internal medicine in the faculty-resident practice, and serves as faculty adviser and mentor to medical residents over the course of their three-year training program.
