If you would like to make an appointment at Behavioral Health Partners, please call us at (585) 276-6900. A representative from BHP will verify the type of University of Rochester insurance that you hold and schedule an appointment for you.

First Visit Paperwork

Prior to your first visit, we will send you additional forms to complete through MyChart. Please complete these forms before your appointment time.

In addition to registration documents, we will ask you to complete a pre-treatment questionnaire to better understand your needs prior to your first appointment.

What to bring to your first visit:

• Your insurance card and photo ID
• A list of current medications
