If you would like to make an appointment at Behavioral Health Partners, please call us at (585) 276-6900. A representative from BHP will verify the type of University of Rochester insurance that you hold and schedule an appointment for you.
First Visit Paperwork
Prior to your first visit, we will send you additional forms to complete through MyChart. Please complete these forms before your appointment time.

In addition to registration documents, to better understand your needs, we ask that you complete a pre-treatment questionnaire before your first visit. You can download the PDF below, fill it out, print (single-sided), and bring it to your appointment. Or, if it is more convenient for you to complete the form in our office, please plan to arrive 15 minutes before your appointment time.
What to bring to your first visit:
- Your insurance card and photo ID
- A list of current medications
