Preparing For Your Visit

Below are some ideas to maximize your visit. This list is provided only as an aid to remind you of what you may want to discuss during your visit.

Create a Visit List That May Include the Following:

  • Your symptoms:
    • What they are
    • When they started
    • Anything that makes them better or worse
  • Your family history of heart disease, stroke, high blood pressure, diabetes
  • Any major causes of stress or recent lifestyle changes
  • Smoking and alcohol history
  • Your activity and exercise routine
  • Your updated medication list including over-the-counter meds and supplements
  • Any special diet you follow

Create Your Question List – Order It from Most Important to Least Important

  • What test(s) will I need?
  • What foods should I eat or avoid?
  • What about physical activity?
  • What's the best treatment?
  • What follow-up appointments do I need?

When Scheduling a Procedure Appointment - Questions to Ask

  • Is there anything I need to do prepare for my appointment?
  • Is my regular diet ok?
  • Do I take all my medications? 
  • Will I be able to drive home or do I need a driver?
  • Do I need to wear sneakers and/or comfortable clothing (for exercise tests)?
  • For your own peace of mind, you may want to check with your insurance carrier to confirm services are covered. Each patient’s insurance coverage may vary.
