Accessing Your Records
MyChart, Your Medical Records, and You
Your MyChart account gives you access to much of the information in your UR Medicine and Affiliates patient medical record. This is available directly through the website and/or mobile app. Patients over the age of 18 are now able to request records not available directly through MyChart; simply submit a 'request medical records release' questionnaire listed under the Sharing section of the main menu. This allows you to obtain copies of records as well as send copies to whomever may need them.
Additionally, through "Sharing", you have options to give access to a family member with a one-time link. Or if you’re seeing a non-UR Medicine provider or receiving care out of town, you can provide access to your records, which gives them the ability to review and comment and those notes will be available to your UR Medicine care team. Accessing Your Requested Records
If records are uploaded by the Release of Information Department to your MyChart account, patients can easily access them directly through the Requested Records section of the Document Center. Simply navigate to the main menu, select Document Center under the My Record banner.
Records are uploaded as an Adobe PDF document that is available for patients to view, navigate, print, or save. Please note that once records are uploaded they are only available in MyChart for 30-Business days. If records are needed after the 30 day period; please contact the Release of Information Department to have records re-uploaded for an additional 30 days. There is no limit to how many times records can be uploaded to MyChart.
Where to Locate Records in Your MyChart
Printing of records works best through web application.
The majority of patient visit notes can be found attached to your appointments and visits. To access these navigate to the Appointments and Visits screen (either by top hand tool bar of home screen or through the main menu under the My Record subsection).
To View and/or Print Visit Notes
Select View notes; this will pull up any notes associated with that encounter that were shared to mychart for the selected date of service. Then, simply click the printer icon in the top right hand corner of the note screen.
These can be found under the Test Results screen. Access this screen by clicking the "Test Results" button via the top tool bar on your MyChart home screen, or navigating through the main menu under the My Record subsection.
To View and/or Print the Results
Click on the corresponding testing in which you wish to view and/or print.
Click on the printer icon at the top right hand corner of the result window that appears.
A list of current and/or past medications can be found under the Medications screen. To access this screen you can click "Medications".
A list of your electronically available immunizations can be found by navigating to the My Record section of the Main Menu and selecting Immunizations.
To view and/or print this list, simply select the Printer icon in the top right hand corner of the screen. If no items are found here, you will need to reach out to the Release of Information Department directly to request older records be checked for any possible Immunization Summaries.

Contact the Release of Information Department to inquire if should be visible and if not, can direct you how to request it be added.
See Something That Should Be updated?
Contact your provider to inquire. All UR Medicine and affiliated treatment providers have the capability to update personal medical history, medications, problem list, and more. If you do not have an active UR Medicine and/or affiliated provider, please contact the Release of Information Department office for assistance.
