Requesting Medical Records

Request Medical Records Through MyChart

Patients can request their records be sent anywhere directly from their MyChart account! To submit a request directly from your MyChart; simply go to the Main Menu drop down and select the option to "Request Medical Record Release" from the Sharing sub-menu.

Need to request a full copy of your records? No problem! The Release of Information Department can provide you an entire copy of any visit you need. This consists of all notes, labs, imaging, flowsheets, and more!

Don’t need a full copy of your records? We can assist with that too! The Release of Information Department can provide an abstract of any visit you need. This consists of pertinent notes (ie: History and Physical, Discharge Summary, procedure/operative notes) in addition to recent labs and imaging results.

  1. Log into MyChart account either via mobile app or website.
  2. Navigate to the main menu.
  3. Scroll down to Sharing section.
  4. Under the sharing section you will see the "Request Medical Record Release" option.
  5. Answer all indicated and necessary areas within the questionnaire.
  6. Once completed you will be asked to review your answers prior to submission. Once reviewed, select the Submit button to send your request directly to the Release of Information Department for processing.

Did you know that the Release of Information department is able to send your medical records anywhere in the world? All we need is the full contact information in which the records need to go!

Full contact information consists of:

  • Full name of receiving party (person, provider, company, etc.)
  • Full mailing address (to include mail codes)
  • Email address (if applicable)
  • Phone number for receiving party
  • Fax Number (to include out of country area codes if applicable)

Please complete the Patient Access form. You may also mail, fax, or email us a signed, letter that includes:

  • Patients name and date of birth
  • Which records you are requesting
  • Where to send the records (name, address, phone number, and email address [if applicable])
  • Requesters contact information should we have a question about the request (your phone number and address)
  • Sign and date the letter

*Please only submit your request via one method to avoid duplicate submissions*

Please complete the SH48 form (SH48 Español) or an OCA 960 form (OCA 960 Español), and mail, fax, or email to The Release of Information Department  for processing.
