ADC Program

The Memory Care Program is one of 18 official dissemination sites for the Alzheimer’s and Dementia Care (ADC) Program, delivering a nationally recognized, team-based dementia care model. 

Through coordinated clinical support, individualized care planning, and caregiver guidance, the program connects patients and families with the services and resources needed to manage changing care needs.

GUIDE (Guiding an Improved Dementia Experience) Model

The GUIDE Model is a care program designed to support people living with dementia and the family members who care for them. By bringing together healthcare providers, community services, and caregiver support resources, the program helps individuals remain safely at home longer, improves coordination of care, strengthens caregiver support, and works to reduce avoidable hospital visits while improving overall quality of life.
