University of Rochester Medicine contracts with most major health insurance carriers. Please call your insurance company and ask if you have access to health care services at URochester Medicine locations, and what (if any) co-payments, co-insurances, and deductibles will be your responsibility.

If you do not have health insurance coverage, several programs are available through New York State, including:
- Medicaid - a program for New Yorkers who cannot afford to pay for medical care
- Child Health Plus - a plan specifically for children under the age of 19
- New York State of Health - New York State's health insurance marketplace that offers low-cost, quality health insurance plans

If you received a URochester Medicine statement with no Insurance Payments applied to your services, please complete all of the fields below to notify our URochester Medicine Patient Services team. Your insurance and charges will be reviewed and new statements issued when needed.
