Mothers and Babies Health Survey 3.0

Your birth story matters.

Giving birth is a powerful, life-changing experience. Monroe County health leaders want to get a better understanding of the experiences and health outcomes of birthing people in our community.

The Mothers and Babies Health Survey 3.0 is an opportunity for you to share your story, your challenges, your joys, and everything in between. By learning directly from your experiences, Monroe County can strengthen maternal health services, improve birthing experiences, and help families feel more supported before, during, and after birth.

Who can participate? 

Birthing people who welcome a baby in 2026 in Monroe County.

What will I be asked to do?

You will be invited to complete an anonymous survey about your pregnancy, birth, and early postpartum experience. Our goal is to fully capture your story, therefore the survey will take about 20-30 minutes to complete.

What do I receive for participating? 

As a thank you for taking the time to share your story, and help us learn about birthing experiences, everyone completing the survey will have the chance to win $100 and receive a special gift for you and your baby.

How do I take the survey?

In the weeks following your baby's birth, you will receive a mailing from the Monroe County Department of Public Health with your baby's birth certificate. The mailing will include a special code and instructions on how to access the survey.

The survey can be completed on your phone, tablet, or computer, at a time that works best for you. If you would like the survey in paper form, or in a different language, please contact us and we will do our best to accommodate your needs.
