The Memory Care Program provides diagnosis, treatment, and care of neurodegenerative conditions that affect the elderly and the aging nervous system.

Our program specializes in treating older patients with:

  • Memory loss
  • Dementia
  • Cognitive dysfunction
  • Alzheimer’s disease

What to Expect from the Memory Care Program

We offer a specialty service focused on patients with conditions affecting the aging brain and older adults. Your first appointment typically lasts about 90 minutes and is divided into three parts. Family members are encouraged to attend the initial visit, as this is a good time to ask questions and to provide input.

During the first part of the visit, a patient care technician will record vital signs, review your current medications, and conduct cognitive screening utilizing a variety of measures. Patients should bring a current medication list to this appointment. During this time, the person accompanying you may meet separately with the physician to provide additional history or discuss concerns.

In the second part of the visit, the patient meets with the physician to review symptoms and medical history and to complete a physical examination. In the final part of the visit, the physician will review the evaluation results with the patient and their companion and discuss the recommended next steps.

Our program provides our patients with:

  • Assessment, evaluation, and diagnostic workup
  • Ongoing medication management and help with challenging behaviors
  • Recommendations for treatment
  • Information, education, and support for their care partners
  • Referrals for community resources
  • Opportunities to participate in research studies

The Memory Care Program is one of 18 official dissemination sites for the Alzheimer’s and Dementia Care (ADC) Program, delivering a nationally recognized, team-based dementia care model.

Through coordinated clinical support, individualized care planning, and caregiver guidance, the program connects patients and families with the services and resources needed to manage changing care needs.

The Memory Care Biologics Treatment Center is designated as a Center of Excellence by the Alzheimer’s Quality Initiative. We prescribe and provide advanced treatment options for eligible patients, including anti-amyloid therapies such as lecanemab and donanemab.

To begin biologics treatment, a confirmed diagnosis of Alzheimer’s disease with biomarker verification is required. If you need to have Alzheimer’s Disease verification, you will first need to see one of our Memory Care Specialists for a comprehensive evaluation.

UR Medicine Memory Care launched the Capacity Clinic to provide psychiatric assessments that determine whether a person currently has the ability to make certain types of decisions.

These evaluations help clarify whether someone has the decision-making capacity required for specific legal, financial, or medical matters.

The UR Medicine memory care program offers unique, innovative, and multidisciplinary memory care to our patients. Our team of experts includes neurologists, geriatric psychiatrists, a geriatrician, a neuropsychologist, a family and marriage therapist, and social workers.

Together, these clinicians help patients and their families deal with the many dimensions of the disease and phases of treatment, beginning with an early and accurate diagnosis. The team meets regularly, provides follow-up and supportive care, and offers access to clinical trials as appropriate.
