Updating Your Information

If you have an appointment in the next few weeks with a UR Medicine provider, you may receive a phone call from one of our Registration and Insurance Management specialists to update and finalize your information in our system.  This allows us to input all the information we need in our system before you arrive for your appointment, saving you time at our check-in desk. 
When calling, all of our specialists should provide you with the date and location for your upcoming appointment to verify their affiliation with UR Medicine.  Some of the information you may be asked for includes:

• Full name
• Address
• Phone(s)
• Date of Birth
• Last four of Social Security (this is voluntary)
• Insurance information
• Emergency contact information
