University of Rochester Medicine network providers may refer patients to our clinic by choosing "Ambulatory Referral to Preventative Medicine" under Department Specialty. Please be sure to select the specific program you're looking for (i.e. tobacco/vaping, nutrition/lifestyle counseling, or the Rochester Diabetes Prevention Program).
