6.1 Uploading Required Paperwork to Box

Each department has a designated Box folder to upload required paperwork for faculty actions. A Faculty Recommendation Form is required for all faculty appointment actions to provide:

  • Clear communication from departments to the Office of Academic Affairs on the details of a faculty appointment/action.
  • Details of an appointment extension: in the remarks, state why there’s an extension and the new end date.
  • Appointment start date, terms, and other information.
  • The date of myURHR initiation (except for Provost/Board of Trustee items that require additional information)

6.2 Onboarding Requirements Upon Hire

  • Intellectual Property Agreement (IPA) – Within myURHR, the new employee will be prompted to acknowledge and sign the IPA.
  • Conflict of Interest Reporting (COI) – Required within 30 days of hire and annually thereafter
  • eCV: Completed by full-time and part-time SMD faculty.
    • New faculty complete within 30 days of hire.
    • Faculty web profiles for URMC are fed by the eCV.
