Be able to knowledgably discuss differences between United States Preventive Task Force Services recommendations for chronic disease (hypertension and cardiovascular disease) prevention and screening and specialty or health-advocacy organization based recommendations. (ACGME Competencies: Medical Knowledge, Practice-Based Learning & Improvement, Interpersonal & Communication Skills)

PMRY1 Objectives

Observe and participate in giving a presentation regarding these factors for one or all of these diseases to fellow professionals, medical students, and/or the general public.

Participate in creating or revising teaching materials to assist with educating health care workers and/or the general public regarding some aspect of prevention.

PMRY2 Objectives

Independently create teaching materials to assist with educating health care workers or the general public regarding some aspect of prevention.

Independently present or lead a discussion amongst your colleagues about the differences between recommendations and the possible reasons for them.

Learn about community-based interventions to encourage recommended health behaviors to prevent diabetes and/or cardiovascular disease. (ACGME Competencies: Medical Knowledge, Practice-Based Learning & Improvement, Interpersonal & Communication Skills, Professionalism)

PMRY1 Objectives

Observe and participate in applying the above knowledge to develop or operate a program to increase activities that reduce the risk of one or more of these diseases and in communicating this plan to stakeholders.

Observe and participate in investigating laws or policies that would discourage activities that increase the risk of these diseases, and/or encourage activities that reduce burden of these diseases.

PMRY2 Objectives

Participate and independently apply the above knowledge to develop or operate a program to increase activities that reduce the risk of one or more of these diseases and in communicating this plan to stakeholders.

Independently and participate in investigating laws or policies that would discourage activities that increase the risk of these diseases, and/or encourage activities that reduce burden of these diseases.

Learn how to build collaborations with various stakeholders and community members themselves to work together on policies or activities to improve the health of the entire community (ACGME Competencies: Medical Knowledge, Practice-Based Learning & Improvement, Interpersonal & Communication Skills, Professionalism)

PMRY1 Objectives

Observe and participate in community collaborative meetings with members of the community exploring or developing a specific activity or policy to improve the health of the community.

Observe and participate in community collaborative meetings on initiatives to improve a specific aspect of the community’s health with leaders of the community and business leaders.

Observe and participate in meetings of the CCH staff and/or leadership with various departments of the medical center or various faculty members to assist them in their outreach to the community.

PMRY2 Objectives

Participate in community collaborative meetings with members of the community exploring or developing a specific activity or policy to improve the health of the community.

Independently present information to a community collaborative meeting to assist with their activities.

Participate in community collaborative meetings on initiatives to improve a specific aspect of the community’s health with leaders of the community and business leaders.

Independently present information at such a meeting to assist with their activities.

Learn about and participate in community-wide efforts to decrease hospital re-admission rates (ACGME Competencies: Medical Knowledge, Practice-Based Learning & Improvement, Interpersonal & Communication Skills, Systems-Based Practices, Professionalism)

PMRY1 Objectives

Observe and participate in staff meetings with the part of CCH responsible for tracking these diseases in the community.

Talk with leadership of CCH regarding the programs and grants available for tracking theses diseases.

Participate in data gathering activities for filing a case report of one of the diseases tracked

Participate in analyzing data evaluating this effort.

Assist with presenting either education to staff involved with this effort or data evaluating the effectiveness of efforts so far to staff or other stakeholders in this effort.

PMRY2 Objectives

Independently analyze data evaluating this effort.

Independently present either education to staff involved with this effort or data evaluating the effectiveness of efforts so far to staff or other stakeholders in this effort.  (Alternative is to this for another major community-wide project led by FLSHA).

Optional: Independently perform your capstone project or MPH thesis using data from CCH’s tracking of infectious diseases in the community.
