For Patients and Caregivers

We provide a need based palliative care service for patients with chronic neurological disease at any stage of illness.

  • Our team can help with a wide variety of issues including:
  • Coming to terms with a new diagnosis
  • Difficult emotions such as sadness, grief, worry or guilt
  • Existential or spiritual challenges such as hopelessness or loneliness
  • Hard to control symptoms such as pain, fatigue and depression
  • Finding support for caregivers and family
  • Communication challenges within a family
  • Planning for the future
  • Assistance around end-of-life or hospice


What to expect:

New patient appointments typically last 2-4 hours
You will meet with multiple members of our team

  • Provider, chaplain/therapist, social work, nursing and often students


Self-referral process

  • We do accept self-referrals
  • We will need updated medical history including previous provider notes, laboratory tests and imaging reports
  • Sometimes there are issues with patients with Medicaid/Medicare should they need transportation if self-referred
