Internal Referrals
Through Epic, start an order for an AMB REFERRAL TO NEUROLOGY, and under subspecialty choose Neuropalliative Care.
External referrals
Primary care providers can e-mail the Neuro Supportive Care team for referrals at: Neuro_Supportive_Care@URMC.Rochester.edu
When sending a referral, please be sure to include the following:
- Patients full name/ date of birth
- Patient address and phone number
- Insurance Information
- Reason for referral
- Any clinical notes to support reason for referral including lab results and imaging reports
We will review the referral and contact the patient within 48-72 business hours.
