Making An Appointment

We know you have choices in selecting a physician for your orthopaedic needs, and we thank you for trusting UR Medicine. Here is the information you need before your appointment.

We see patients by appointment only. Please call the appropriate appointment number and our staff will help get you scheduled. If you use our online form, someone will call you within one business day.

We will need the following information:

  • The name of the physician referring you
  • Your medical insurance card
  • Your physician referral number (if your insurance requires it)
  • Your Workers' Compensation carrier name and case number, if you have an injury that happened at work
  • Your auto insurance carrier's phone number and case number, if you were in a motor vehicle accident

Tell us about special needs
If you (or the patient you're calling for) need an interpreter, have mobility issues, or will need other special assistance, please let us know in advance. We will do our best to accommodate you.

Help us stay on schedule
Please be on time for your appointment. This helps us stay on schedule. If we are delayed and you have to wait longer than we intended, rest assured that you still will get the personal attention you need during your appointment.

Give us 24 hours
If you must postpone or cancel your appointment, please let us know at least 24 hours in advance.
